State of HIV in Peru
In short, the epidemic remains concentrated with a prevalence in the general population of 0.23% (2002 sentinel surveillance of expectant mothers) and 12.4% in men who have sex with men (MSM) (2001 sentinel surveillance). According to the national reporting system, 2,895 new cases of HIV and 1,051 cases of AIDS were reported in 2011. Since the first case of AIDS was reported, the reporting rate rose until 2005 (with some fluctuations), and then reversed to a downward trend. Likewise, there was an upward trend in reported cases of HIV until 2008, when the rate began decreasing steadily until 2011. The male-female ratio over the decade of 2000-2009 has remained stable, at a value of 2.8 for 2000-2004, and 2.7 for 2005-2009. The largest number of AIDS cases between 1983 and 2011 were found in the cities of Lima and Callao (71%), both of which account for 34.1% of the national population. The main mode of transmission continues to be sexual (97% of cases), followed by vertical (2%) and parenteral (1%).
The epidemic in Peru is concentrated. According to the results of the sentinel surveillance conducted in Peru in this population group, in 2002 there was an incidence rate of 8.3 new cases per 100 person-years (IC95% 4.5 to 14.7), in 2006 it was 3.1 (IC95% 1.0 to 5.3%), and in 2011 the rate was 5.2 (3.8 to 6.5). A fact that stands out in the 2011 surveillance is that the incidence rate in MSM under the age of 25 rose from 6.2 in 2002 to 9.6 in 2011. While point estimates vary each year, confidence intervals overlap, indicating that the incidence rate in this group has remained stable over the past 10 years. Regarding the prevalence in the MSM population, it was estimated at 22.3% in 2002, dropping to 11.1% in 2006, and 12.4% in 2011. In the case of the transgender population and MSM sex workers, the highest prevalence was recorded in 2002 (29.2%), dropping to 4.05% in 2006, and rising to 14.6% in 2011. In the case of the transgender population, the prevalence in the last sentinel surveillance reached 20.8% (104/500), with an incidence of 9.07 per 100 person-years, making it the most affected and highest risk sub-group.
